On or about October 29, 2014 west the east side of the structure, an employee used a wooden job made ladder without filler blocks inserted between the rungs to support the cleats, exposing himself to a 9-foot 5-inch fall hazard. Among other methods, one feasible and acceptable method to correct this hazard is to, but not limited to the following: (1) conform to the American National Standard Institute (ANSI) A14.4-1992, section 4.5.2. |
