Crossbraces shall not be used as a means of access. On or about 08-24-12 at 77521 Overseas Hwy Islamorada FL, 33036, a fabricated frame scaffold was not provided with means of access while employees accessed the scaffold by climbing the frames of the scaffold, exposing employees to a fall hazard of approximately 22 feet to ground. |
