On or about March 29, 2023, in the machine shop, the employer exposed machinist employees to amputation hazards in that the 55 Ton Ironworker did not have a punch guard at the point of operation. b. On or about March 29, 2023, in the machine shop, the employer exposed machinist employees to an amputation hazard in that the Turn-N-Bend press foot actuator pedal, that is integral to guarding the point of operation, was missing the pedal cover intended to prevent unintended operation. |
