On or about November 1, 2016, at the above referenced job site, employees who were working below/around supported scaffolds and were exposed to head injuries from hand tools, such as but not limited to rebar, which were being used by employees working on the second and third tier of the scaffold, which were not equipped with toe boards. |

The employee was measured for 283 minutes with a zero dose assumed for any un-sampled period. |
