Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about 06 March, 2015, on the above addressed job site, nine workers were installing the fiber glass and insulation on the roof of an existing 9 story apartment building, without the use of fall protection, exposing the workers to a fall hazard of up to approximately 50 feet in height. |
