Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: On or about January 15, 2015, at the above addressed jobsite, employees were exposed to a fall of approximately 15 feet while removing an existing roof to replace it for a new roof at a commercial construction site without fall protection.
