Citation text line 29 CFR 1910.212(a)(3)(ii): Point of operation guards were not designed and constructed as to prevent the operator from having any part of their body in the danger zone during the operating cycle: a. On or about June 9, 2016, in the packing house, employees working on the AMI watermelon line were not protected from an amputation hazard, in that there was no guarding over the 3-belt pulley system as the machine was in operation, resulting in an amputation. b. On or about June 9, 2016, in the packing house, employees working on the AMI watermelon line were not protected from a caught-in hazard, in that there was no guarding on a shaft that protruded from its respective drum.
