Employees were exposed to an electrocution hazard in that the employees were using electrical tools supplied power through: 1: A defective Ground Fault Circuit Interrupter outlet that was damaged and could not be tested. 2: The extension cord in use for the employees working on the roof was missing the path to ground. 3: The extension cord in use had a cut in the outer insulation approximately 3 inches from the plug end exposing the energized hot conductor.
