Citation text line 29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: a) At the jobsite located at 13290 Corbel Circle in Fort Myers as observed on or about 6 October 2020. Employees were exposed to an electrocution hazard in that the employees were using electrical tools supplied power through: 1: A defective Ground Fault Circuit Interrupter outlet that was damaged and could not be tested. 2: The extension cord in use for the employees working on the roof was missing the path to ground. 3: The extension cord in use had a cut in the outer insulation approximately 3 inches from the plug end exposing the energized hot conductor.

Note: As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). On or about July 20, 2015 at the above addressed job site, an employee performing roof metal sheeting operation working from an aerial lift was not attached to the boom, exposing him to a fall hazard of approximately 26 feet.
