At the jobsite, on the roof, fall protection was not provided to an employee installing metal sheathing on the top of a parapet wall, exposing the employee to a 30 foot fall hazard, on or about 6/15/2016. b. At the jobsite, on the roof above the drive thru window, fall protection was not provided to an employee installing a conductor head as part of the drainage system, exposing the employee to a 10 foot 9 inch fall hazard, on or about 6/15/2016. West side of building - employees were exposed to a 11'-2" fall hazard, in that, the use of fall protection was not enforced while working from the flat portion of the roof near the edge.
