29 CFR 1910.147(c)(7)(i)(A): Authorized employee(s) did not receive training in the recognition of applicable hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control. a) For the manufacturing facility as observed on or about 10/31/2017, authorized employees were not trained on lockout tagout procedures. Authorized employees were not aware of all the required steps of lockout tagout and were exposed to hazardous energy in the form of 240/480VAC electricity, hydraulic energy, and moving parts while performing maintenance on equipment such as, but not limited to, hydraulic press brakes. Maintenance work performed included, but was not limited to, cleaning in the back of presses, greasing the rails of the presses, changing hydraulic lines for presses, electrical troubleshooting, and fixing leaking valves. |

29 CFR 1910.147(d)(3): All energy isolating devices that were needed to control the energy to the machine or equipment was not physically located and operated in such a manner as to isolate the machine or equipment from the energy source: For the manufacturing facility, as observed on or about 10/31/2017, maintenance/servicing work was performed on hydraulic press brakes without de-energizing the presses. The presses contained 480 VAC electricity, hydraulic lines, and moving parts that created pinch points. a) Greasing of hydraulic rails in the back of the presses was conducted without de-energizing the press; rather, only an E-stop was actuated. b) Cleaning in the back of the presses was conducted without de-energizing the press; rather, only an E-stop was actuated. c) Die changing activities were conducted without de-energizing the presses. |
