On or about October 10, 2017 employees were working on a 5:12 pitch roof to install truss blocking and bracing and had utilized a 6 foot, Werner brand fiberglass A frame step ladder to gain access to the top plate of the CMU wall 9 feet 4 inches above the ground. The step ladder was too short for its intended use and the side rails of the ladder did not extend 3 feet above the surface being accessed, nor was it secured to prevent deflection while climbing the ladder.
