The respirator was left open on the hooks and had visible dust on the outside and inside of the respirator. This violation was observed on or about May 20, 2015. b) In the blast booth, 3M 6000 series Half-face Dual Cartridge Respirators, Bullard M-100 Series Blasting helmets and Nova NV3-750 Blasting Helmets are not protected from dust, in that, the respirators were stored within 3 feet of where employees remove clothing that contains silica, Hexavalent Chromium, Beryllium, Cadmium and Lead. The respirators had visible dust on the outside and inside of the respirator. This violation was observed on or about May 20, 2015. |
