Citation text line OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to overhead struck-by hazards: Instance 1: On or about February 24, 2017, at the above addressed jobsite, employees were exposed to overhead struck-by hazards from merchandise being stored on a 3-tier shelving unit that was not plumb or anchored to the ground with anchor bolts. Instance 2: On or about February 24, 2017, at the above addressed jobsite, employees were exposed to overhead struck-by hazards from merchandise being stored on racks of a 3-tier shelving unit that did not have load ratings present to provide, at a minimum, the maximum permissible unit load and/or maximum total load per bay. Instance 3: On or about February 24, 2017, at the above addressed jobsite, the employer did not develop a maintenance and inspection program for storage racks, which includes keeping aisles clear and providing sufficient clearance for material handling equipment, ensuring racks are properly aligned, leveled, plum, and anchored per manufacturer's instructions, as well as encouraging employees to promptly report any damages to racks. NOTE: ABATEMENT CERTIFICATION AND DOCUMENTATION REQUIRED FOR THIS VIOLATION.
