At 3516 Oystercatcher Way, Jacksonville, FL 32224; On or about September 9, 2022, and at times prior, employees were exposed to burns and / or electrocution hazards in that extension cords used at the job site were not visually inspected each day, and two of the extension cords were missing a grounding prong. |
