At the job-site located at 4962 Distribution Dr., Tampa, Florida: On or about November 1, 2022, employees were exposed to fall hazards of 25-feet while working on the roof of an industrial structure with steel roof deck, performing insulation and thermoplastic polyolefin (TPO) removal and installation. The roof had a 1:12 pitch; 25-feet to the eave. There were no guardrails, safety net, positioning devices, fall restraint systems or personal fall arrest system. |
