On or about 03/15/2023, at the jobsite at 509 65th St, Holmes Beach, FL., employees were exposed to fall hazards of approximately 9.4 feet, in that, the Werner fiberglass 24-foot extension ladder used to access the 2nd floor only extended above the landing surface approximately 24 inches.
