On a 4:12 pitch roof, at a worksite located at 8513 Howell Drive, Jacksonville, Florida 32208: On or about February 15, 2024, the employer exposed five employees to fall hazards, in that, fall protection was not utilized by employees while nailing down decking and installing roof shingles. The employees were exposed to falls hazards ranging from 8 feet to 16-feet. |
