At a residence, located at 4801 W Leona St, Tampa FL: On or about 9/17/2022 the employer exposed employees to fall hazards, in that the ladder in use from the first roof to the second roof landing did not have side rails extending at least 3 feet (.9 m) above the second roof landing surface, the railings were approximately 1 foot above the roof edge.
