On or about 07/29/2020, at 3800 Deer Drive, Sarasota, FL 34240, employees were exposed to a fall hazard while working from a tubular welded frame scaffold 4 frames high and 7 frames wide, in that, the scaffold was not provided with guardrails as required. The scaffold top frames (fourth frame) were missing the guardrails at the ends and at 6 other frames, the third frames were missing the guardrails at the ends and at 3 other frames and the second frames were missing the guardrails at the ends and at 3 other frames. |
