On or about November 19, 2020, on the rooftop of a residential structure in construction, an employee performing sheathing and framing work on a roof was not protected by any means of personal fall protection, guardrails or safety nets, thereby, exposing the worker to a fall hazard of 8 feet, 1 inch to ground level. |

29 CFR 1926.404(b)(1)(iii)(C):Where an assured equipment grounding conductor program was being utilized, the employer did not ensure that each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, were not visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. a. On or about August 12, 2020, and at times prior, employees were exposed to 115 Volt electric shock hazards in that extension cords used at the job site were not visually inspected each day, and one of the extension cords was missing a grounding prong and had damaged insulation. |

On or about August 12, 2020, and at times prior, at the edge of the second floor at the open stairwell, employees were exposed to a 10 feet fall hazard in that there was no guard rail system, nets, or use pf personal fall protection equipment to protect employees from the fall hazard. |
