While repositioning the cover, one of the employees pulled up on handle forcibly causing the handle on the opposite side to go down rapidly pinching the end of the employee's middle finger between the cover's handle and the silo roof. This caused a laceration to the finger and the loss of the fingernail.

Narrative Employee was using a jackhammer to demo a concrete step. When the concrete broke free, the jackhammer bounced hitting the employee's left boot at the edge of the steel toe. Narrative A portable welding generator was operating outside of the Auxiliary Mill Control Room. The diesel exhaust fumes were drawn into the control room and accumulated. Narrative A sledge hammer was used to remove a sprocket segment from a bucket elevator. When the sledge hammer hit the sprocket, metal came off and hit an employee about 15-20 feet away in the abdomen. The employee was taken to the ER to receive an X-Ray to verify no metal remained in the laceration, a tetanus shot, and two stitches before being returned to work.
