Citation text line 29 CFR 1926.454(a): The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazards: a) On or about 08/14/2019, at the construction site, an employee working in an aerial lift at a height of 21 feet was utilizing a fall arrest system connected to a 23 foot rope with no deceleration device. The employee had not been trained on the correct way to setup a fall arrest system. b) On or about 08/14/2019, at the construction site, an employee working in an aerial lift at a height of 21 feet was wearing his harness' chest strap at his stomach. The employee had not been trained on how to wear the harness.
