Crossbraces were used as a means of access. a) On or about February 23, 2022 and times prior; residential construction site at 330 East Sunset Ave, Pensacola, FL: The employer failed to ensure employees conducting work activities from a fabricated tube scaffold had proper access on/off scaffold platforms in that employees climbed the end frames and cross bracing due to no adequate ladder being set-up, exposing them to fall hazards of various heights.
