Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system. a) On or about July 24, 2018, the employer failed to ensure that employees conducting roofing activities on the flat roof of the building locate at 1260 Palm Coast Pkwy SW in Palm Coast Florida, working and walking exposed to a fall of more than six (6) feet to the ground, were protected from falling by the use of a guardrail system, safety nest system or personal fall arrest system.
