29 CFR 1910.134(c)(2)(i): Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible: a. On or about October 19, 2017, the employer provided and had available for voluntary use by the employees N95 dust/particulate respirators and had not provided them with the 29 CFR 1910.134 Appendix D mandatory information. OTR Wheel Engineering, Inc. (formally known as Long's Wheel & Rim, Inc.) was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1910.134(c)(2)(i), which was contained in OSHA inspection number 1057178, citation number 2, item number 1-a and was affirmed as a final order on August 4, 2015, with respect to a workplace located at 940 Eastport Road, Jacksonville, Florida 32218. |

On or about October 19, 2017, inside the facility spray paint booth, CSHO observed an employee (Painter) using a spray paint gun that was discharging flammable paints and thinners with no visible gauges, audible alarms or pressure activated devices installed in paint spray booth to indicate or ensure that the required air speed was maintained to prevent the accumulation of flammable vapors, thereby exposing the employee to fire and explosion hazards. |
