On or about April 27, 2017, at the first level rooftop, employee conducting framing operations on a 3:12 pitch roof was exposed to a 8 feet fall hazard to ground level without the use of personal fall arrest system, safety net system or guardrail system. c. On or about April 27, 2017, at the second level rooftop, employee performing roof sheathing on a 6:12 pitch roof was exposed to over 6 feet and higher fall hazard to lower level without the use of personal fall arrest system, safety net system or guardrail system. d.
