(a) Back of house - employees were exposed to a fall hazard of up to 14 feet, in that there was no safe access. The employees were using the end frames and they were going from the top of the scaffold and climbing over the 2nd floor guardrail to access the 2nd floor of the house, on or about 8/11/17.
