On or about August 16, 2017, on the east side of the building, employees performing concrete works from a swing stage scaffold were not protected from a struck-by hazard when the wire rope snapped sending the platform and materials against an employee suspended from his fall protection system. The employer did not inspect the wire rope cable prior to each work shift neither replaced it as it was corroded. |
