Employees were observed installing roof sheathing on a 6:12 pitch roof. The eave to ground height was approximately 10 feet 7 1/2 inches and the employees had no fall protection system in place.

Citation text line 29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a) For employees working at 28093 Wicklow Court in Bonita Springs, as observed on or about March 8, 2016. Employees were using three ladders to access the upper level landing surface. All three of the ladders were not extended a minimum of 3 feet above the upper landing surface. Two of the ladders extended approximately 1 foot above the landing and the last ladder was flush with the fascia board.
