The Integrity Roofing & Gutters was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(B)13, which was contained in OSHA inspection number1117109, citation number 1, item number 2 and was affirmed as a final order on 02/19/2016, with respect to a workplace located at Jacksonville Florida. |

On or about January 11, 2016, at the west side of building #8, the Werner 12-foot aluminum ladder placed on the canopy roof only extended 6 inches above the landing surface and it was not secured at the top nor there was a grab-rail, exposing employees to a 12ft-9in fall hazard to the ground. |
