At 5628 Ocean Blvd. (Lot 3), Fernandina Beach, Florida: On or about October 31, 2022, the employer exposed employees to a cave-in and engulfment hazard, in that, employees setting up the footing for a residential building were allowed to work right below an unsupported excavated sand dune wall that measured about 14 feet high. |
