Employees were installing framing members for the roof of the new construction residential home without the use of any fall protection. Employees were exposed to a fall of approximately 21 feet 2 inches.

Employees were working on second floor of the new residential home with window openings that were unguarded. Employees were exposed to a fall of approximately 14 feet. The window opening inside edge measured 26 inches.
