29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure and employee training to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated, and rendered inoperative in accordance with 29 CFR 1910.147(c)(4): On or about June 27, 2016, and at times prior thereto, at the above addressed worksite, the employer did not develop, implement and document an energy control procedure (lockout/tag out program), conduct periodic audits and did not provide training on the energy control procedures to ensure that all required elements of the procedure were understood by employees and that the knowledge and skills required for the safe application, usage and removal of the energy controls were acquired by employees performing servicing (lubrications of gears and cleaning machines) and repair work on a machine or equipment, such as; but not limited, to bread ovens, bread sorters and mixers. |
