(1) On or about January 14, 2015, at the above addressed job site, employees using a STIHL TS-420 Cutoff Machine were not provided with eye protection, exposing them to flying projectiles and silica dust that can result in corneal abrasion and laceration to the eye. (2) On or about January 14, 2015, at the above addressed job site, employees using VP 1550 Low Vib Asphalt/Soil Plate Compacter were not provided with eye protection, exposing them to flying projectiles that can result in corneal abrasion and laceration to the eye. |
