Employees installed a tubular frame scaffolding to hoist material to the upper working surface that was not erected or inspected by a competent person qualified in scaffolding erection. Tubular frame scaffolding was missing base plates and did not have fall protection installed on all sides to exposing employees to a fall hazard of approximately 12 feet.
