Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) At the jobsite, employees in the process of roofing activities on a 4 in 12 slope (vertical to horizontal) roof, were exposed to a 8'11" foot fall hazard, in that, fall protection equipment was not installed or utilized, observed on or about 07/10/2014. |
