On or about April 23, 2015, at the Arium Apartment complex, employees who had not been trained by a scaffold competent person were allowed to reconfigure scaffold components while working from scaffolds to install windows, and these employees were not able to recognize hazards such as incomplete planking, inadequate guardrails, and excessive distance from the front of the scaffold to the working surface. |
