Employee was exposed to a 10-feet 6-Inches fall hazard. b) The employer fails to ensure effective protection against falls as required by the standard, as demonstrated on or about January 26, 2018 at 120 Manor Lane Lot # 116 Saint Johns, FL: When an employee was observed installing roof underlayment on the front porch roof at a height of 11-feet and a roof pitch of about 2:12 and the employee was not protected by guardrail system, safety net, or personal fall arrest system, nor was employee provided with an alternative fall protection measure. Employee was exposed to a 11-feet fall hazard.
