The scaffold was not fully planked, in that, it was 40 inches wide with one 10 inch wood plank and one 19 inch metal plank. b) West side - 2nd level - an employee was exposed to a fall of 13 feet 3 inches while using the scaffold to determine if the stucco was dry. The scaffold was not fully planked, in that, it was 40 inches wide with three 10 inch wood planks. c) West side - 3nd level - an employee was exposed to a fall of 19 feet 7 inches while using the scaffold to determine if the stucco was dry. Crossbraces were used as a means of access. a) North side - behind the patio - 2nd level - an employee was exposed to a fall of 13 feet while using the end frames to access and egress the scaffold. b) West side - 2nd level to the 3rd level - an employee was exposed to a fall of approximately 6 feet while using the end frames to access and egress the scaffold.

There was no guardrail system on the north end of the scaffold. c) On or about October 19, 2012, an employee was exposed to a fall of 19 feet 7 inches while using the scaffold to determine if the stucco was dry. There was no guardrail system on the north end of the scaffold.
