On or about July 2, 2018, two roofing employees that were exposed to 12-14 feet fall hazards set up fall protection systems with anchor plates that were not correctly installed per manufacturer instructions in that there were an insufficient number of fasteners installed, the plates were damaged, the plates were not flat against the roof surface, and the fasteners were not the correct type, reducing the pull strength of the anchor. |
