On or about May 6, 2021, at the site located at 17210 SR 438, Oakland, FL, an employee performing siding work from a JLG Model 360SJ was not protected from an approximately 15-foot fall hazard when a lanyard was not worn. The Tower Stucco, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.453(b)(2)(v), which was contained in OSHA inspection number1232535, citation number 1, item number 2, and was affirmed as a final order on July, 27, 2017, with respect to a workplace located at 512 Flagler Avenue, New Smyrna Beach, FL . |
