At the roof, employees were exposed to a fall hazard over 20 feet high, from the a scaffold erected on the roof, to the ground below, in that, a single frame scaffold had a single plank platform bridged over the parapet wall of the roof without guardrails and without any other means of fall protection implemented. b. At the north side of the building, employees were exposed to a 16 feet fall hazard, in that, a tubular welded scaffold two frames high was used to reach elevations at the exterior wall while installing Styrofoam and standing on a single plank platform erected over bundles of Styrofoam on the top scaffold frame. The railings were not raised or a section was not added to the existing railings thus exposing employees to falling over the top rail. c. At the north east corner of the building, employees were exposed to a 20 feet fall hazard, in that, while installing Styrofoam, a platform was used without guardrails while raised on the forks of a Lull JLG fork lift. The platform was a pallet raised on the forks of a Lull JLG 644E-42 forklift.
