29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles: a) An employee operating a debris blower is not being protected from eye injury hazards by the appropriate eye or face protection: The employer fails to assure effective protection against eye or face hazards from flying particles, as required by the standard, as demonstrated on or about August 1, 2013, when an employee blowing debris from a roof of a residence, located at 802 McDuff Avenue South, Jacksonville, Florida 32205, is exposed to struck-by hazards, in that eye protection is not utilized. Father and Son Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.102(a)(1), which was contained in OSHA inspection number 1433729, citation number 02, item number 001 and was affirmed as a final order on May 21, 2020, with respect to a workplace located at 5577 Old Middleburg Road, Jacksonville, Florida 32210. |

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): a) Employees performing roofing operations at elevations of six feet or more above the ground are not being protected from fall hazards by a fall protection system: The employer fails to assure effective protection against falls as required by the standard, as demonstrated on or about August 1, 2023, when three employees were removing the existing roofing system from a residence located at 802 McDuff Avenue South, Jacksonville, Florida 32205, while exposed to the hazard of a 16-foot fall to the ground, in that no form of fall protection systems are in place or in use. Father and Son Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1433729, citation number 01, item number 001 and was affirmed as a final order on May 21, 2020, with respect to a workplace located at 5577 Old Middleburg Road, Jacksonville, Florida 32210. |
