At the perimeter of the building, employees were exposed to a fall hazard up to 25 feet high, in that, a four frame scaffold erected to reach working elevations during exterior stucco work was not provided with standard railings at all areas. 1. At the north side of the building the scaffold had bridging from section to section at the second and third frames without fall protection. 2. At the perimeter of the building the scaffold end sections were open sided. 3. At the entire perimeter of the building the scaffold at the second and third frames had bracing as a top rail but did not have a midrail. On or about 02/08/2017.
