At the Mosaic site located at 4873 Port Sutton Rd., Tampa, FL, employees performing maintenance work were exposed to an ammonia release incident during the process of removing an 8" Orbit valve (MOV P2A) in the anhydrous ammonia system, on or about 12/14/2017. No means were used by the employer to assure that training provided to the employees was understood for the work to be conducted at the site. |
