Employees were using an extension ladder to access the third floor balcony from the second floor balcony that did not have the side rails extended a minimum of 3 feet above the upper landing surface. The ladder side rails only extended approximately 16 inches above the upper landing surface. |

Employees were installing framing lumber and sheathing on the third floor roof of a residential home above 6 feet without the use of any fall protection. The roof pitch was 8:12. Employees were exposed to a fall hazard of over 23 feet. b) For employees working at 203 17th Street in Bradenton Beach, as observed on or about December 11, 2014. Employees were accessing the third floor balcony from the second floor balcony of a residential home. the 2nd floor balcony had unprotected sides above 6 feet without the use of a fall protection system in place. Employees were exposed to a fall hazard of approximately 14 feet. Alex Valerio's Carpentry, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(1), which was contained in OSHA inspection number 912271, citation number 01, item number 001 and was affirmed as a final order on 9 September, 2013, with respect to a workplace located at 6713 Congress Street., New Port Richey, Florida. |
