29 CFR 1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity: a. On or about April 2, 2014, at the above addressed job site, employees who had been performing concrete restoration work from deficient suspended swing stage scaffolds and fabricated frame scaffolds, were exposed to fall hazards as the scaffolds had not been inspected by a competent person before each work shift and had the following deficiencies: 14 Instances: There were 7 ea. swing stage scaffolds that each had a fabricated frame scaffold. 1.The 7 fabricated frame scaffolds were lacking planking, and guardrails. 2. Swing stage scaffolds which had been suspended and secured by wire ropes which had clips that had not been retightened to the manufacturer's recommendations at the start of each work shift. 3. Vertical lifelines that had been used on swing stage scaffolds had been tied to anchorages with knots. 4. Single tiebacks had been installed at angles to the face of the building.
