On or about April 2, 2014, at the above addressed job site, employees who had been performing concrete restoration work from deficient suspended swing stage scaffolds and fabricated frame scaffolds, were exposed to fall hazards as the scaffolds had not been inspected by a competent person before each work shift and had the following deficiencies: 14 Instances: There were 7 ea. swing stage scaffolds that each had a fabricated frame scaffold. 1.The 7 fabricated frame scaffolds were lacking planking, and guardrails. 2.
