29 CFR 1926.20(b)(2):The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person: On the North tower of 2765 Hwy A1A in Indialantic Beach, on or about March 31, 2021, and at times prior, employees who used scaffolds, ladders, aerial lifts, power tools, saws, and directed cranes for truss installations, were exposed to workplace hazards related, but not limited to, electrical shock, lacerations, falls, and struck-by, in that the worksite was not frequently inspected. |

29 CFR 1926.1060(a):The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would train each employee in the procedures to be followed to minimize hazards related to ladders and stairways: a. On the north tower of 2765 Hwy A1A in Indialantic Beach, on or about March 31, 2021, and at times prior, the employer had not trained employees who installed truss brackets from multiple models of stepladders on the 5th floor balconies and along exterior walls, on the procedures to recognize and minimize hazards related to ladders, thereby exposing the workers to fall hazards up to 40 feet. b. On the north tower of 2765 Hwy A1A in Indialantic Beach, on or about March 31, 2021 and at times prior, the employer did not train an employee who installed wood framing from a HaulMaster Multi-task ladder on a 5th floor balcony, in the procedures to recognize and minimize hazards related to ladders, thereby exposing the worker to a fall hazard of 40 feet to ground level. |
