On or about April 3, 2015, at the West Side on the exterior wall, an employee installing floor trusses standing on a top plate was exposed to an 18 feet 9 inches fall hazard to a ground level, without the use of personal fall arrest system, safety net system or guardrail system. b. On or about April 3, 2015, at the West Side on the interior wall, an employee installing floor trusses standing on a top plate was exposed to a 9 feet 2 inches fall hazard to a lower level, without the use of personal fall arrest system, safety net system or guardrail system. |
