On or about October 5, 2012, on a 9-12 pitch roof, the anchorage points were not capable of supporting at least 5,000-pounds (22.2 kN) per employee in that they did not follow the manufacturers instructions for installation of anchors to wooden roofs, exposing employees to a 36-foot fall hazard. |
